General Terms

 

As a CGD member I understand and agree:

 

• A CGD membership is not transferable, and I will not loan my membership card to another individual.

• I will notify CGD of any change in my mailing address or other contact information.

• CGD is unable to accept responsibility for lost or stolen items on CHDC premises.

 

CHDC/CGD Code of Conduct Statement – Our commitment to each other

CHDC/CGD is a shared experience for everyone to enjoy. Each of us can make it better for all by being considerate to others. CHDC/CGD members, staff and volunteers pledge to treat one another with respect and dignity. CHDC/CGD reserves the right to suspend or cancel a membership in its sole discretion for inappropriate behavior or failure to comply with the Code of Conduct Statement.

 

Your safety is important to us

Our CHDC/CGD commitment to proper maintenance and upkeep of our building and equipment is an important part of ensuring a safe environment for all to enjoy. This may result in temporary interruptions in service or access to specific areas or equipment within the facility. Membership fee adjustment(s) shall not apply due to maintenance, repair or refurbishment by CHDC/CGD. In the event the facility is temporarily unavailable, members have the option of rescheduling the session.

 

Limitation of Liability

While CHDC/CGD staff and instructors make every reasonable effort to minimize exposure to known risks, CHDC/CGD recommends that you consult with your physician before participating in any program, and/or using any equipment or facilities. Neither CHDC/CGD nor its staff, instructors or volunteers are licensed medical care providers and have no expertise in determining the effect of any specific program such as an exercise program, or equipment on a medical condition. You understand and agree that in registering for a CHDC/CGD program and/or using any fitness equipment or facilities, you may be involved in physical activity and that with any physical activity there is a risk of injury. By participating in CHDC/CGD programs and/or using any fitness equipment or facilities, you agree that you are participating voluntarily and do so at your own risk and you agree to fully release CHDC/CGD and its officers, directors, agents, staff, instructors and volunteers from all claims or lawsuits for any injuries, death, property damage or theft, losses, or any other liability of any kind, arising directly or indirectly out of your participation in our programs and/or use of any fitness equipment or facilities.

 

CHDC/CGD commitment to privacy

CHDC/CGD is committed to protecting personal information by following responsible information handling practices. We collect and use personal data in order to better meet your service needs, to ensure a safe environment while members are visiting our centre, for statistical purposes to inform you about the our programs in which you are registered, to complete payment transactions and to satisfy regulatory obligations. You may also hear from us periodically about other programs, services and opportunities that may interest and benefit you. 

 

 

 

 

Credit & Cancellation Policy 

 

Camps & Special Programs & clinics

Medical: Planning for all camps and programs is based on current enrolment numbers well in advance of the camp/program start date. As a result no refunds or cancellations are permitted, except for medical reasons. If an athlete is unable to attend the camp/program/clinics (and notice is given prior to the start date) due to medical reasons, he/she will receive a credit for their full amount paid no $25 administration fee will apply, which may be used for any camp/program taking place within the next one (1) full calendar year. A doctor's note is required to substantiate this type of cancellation, and this note must be provided within 1 week of notification or the administration fee will be applied.

 

30 business days Prior: If an athlete is unable to attend the camp/program/clinics (and notice is given 30 business days prior to the start date) for any reason, he/she will receive a credit for their full amount paid less a $25 administration fee, which may be used for any camp/program/clinic taking place within the next one (1) full calendar year.

Within 30 business days: If an athlete is unable to attend the camp/program/clinics (and notice is given within 30 business days prior to the start date) for any reason, he/she will receive a credit for 50% of the camp/special program/clinic fee, which may be used for any camp/program/clinic taking place within the next one (1) full calendar year.

 

 

GLV and PLV Group Sessions

Planning for all sessions (coaches/shooters/ice allocation) is based on current enrolment numbers advance of the session start date/time. As a result cancellations are permitted at anytime for medical reasons. If an athlete is unable to attend the session and notice is given prior to the start date due to medical reasons, he/she will receive a credit for their full amount paid no $25 administration fee will apply, the credit, which may be used for any session taking place within the next one (1) full calendar year. A doctor's note is required to substantiate this type of cancellation, and this note must be provided within 1 week of notification or the administration fee will be applied.  If an athlete is unable to attend the session and notice is given 72hrs prior to the start date for any reason, he/she will receive a credit for the session for their full amount paid no $25 administration fee will apply, the credit, which may be used for any session taking place within the next one (1) full calendar year.  If an athlete is unable to attend the session and notice is given within 72hrs-48hrs prior to the start date for any reason, he/she will receive a credit for the session for their full amount paid less $25 administration fee will apply, the credit, which may be used for any session taking place within the next one (1) full calendar year.  If an athlete is unable to attend the session and notice is given within 24hr/same day prior to the start time for any reason, he/she will receive no credit for the session.

 

 

GLV Semi and Private Sessions

Planning for all sessions (coaches/shooters/ice allocation) is based on current enrolment numbers advance of the session start date/time. Injuries:  In the event of an injury during a camp/program/clinic/private/semi-private sessions no credits will be issued. Planning for all programs is based on current enrolment numbers well in advance of the start date. It is understood that all athletes/parents/guardians voluntary engage in this activity understanding the risks of injury during a camp/program/clinics/private/semi-private sessions.

 

Additional information: Please note the following

 

• No cash refunds will be permitted/issued, we will issue credits to your account.

• Credits issued are transferable from athlete-to-athlete provided the accounts in-question are in good standing and current. All athletes must have or purchase a valid membership card.

• Business days are designated as Monday –Friday within 9-5 business hours. All cancelations must be made by email only. No verbal notifications will be accepted.

• Rescheduling dates prior to a currently booked session within the stated above timeline parameters apply and would be treated as cancelations.
